The Rastelli procedure (named after Rastelli who developed it in
1969) is most commonly used in severe Tetralogy
of Fallot and Pulmonary Atresia/VSD but is also used in Truncus Arteriosus
and TGA/VSD with pulmonary stenosis.
The procedure involves repairing the previous palliative procedure (if
there has been one - eg. closing a BT shunt in pulmonary atresia/VSD) and
then the VSD is closed and the pulmonary
artery connected to the right ventricle using a a homograft (cadaver valve)
or, more recently, a bovine vein graft has been used as an alternative.
The homograft wears out eventually and so requires replacement.
