Shared Care Program

The Paediatric Shared Care (PSC) program aims to provide comprehensive and coordinated quality care to children with chronic diseases Through co-management of children under our care at the NUH The Children's Medical Institute with the primary care doctor/family physician.  The children on the PSC program are each issued a health booklet containing details of the illness, treatment and follow-up plans.

The PSC program operates on three guiding principles:

  • Children under our care are entrusted to your continuing care.  Coordination of their management is maintained throughout telecommunications as well as the follow-up notes contained in the health booklets.

  • Subsequent admissions for acute and specialist care at The Children's Medical Institute are facilitated by the specialists.

  • Upon discharge, these children will be referred back to the primary care doctor for follow-up.

The PSC also provides postgraduate training for the participating doctors in the form of Symposia and the publication of a Bulletin.
